Add the Individual

The agency admin creates the individual's profile and sets up their active ISP (goals and care plan). This is what care will be documented against.

Schedule the Visit

A supervisor assigns a caregiver to the individual on the calendar — picking the date, time, and service type for the visit.

Service Logs

At the scheduled time, the caregiver opens CareNotes on their phone, fills in the guided note (interventions, progress, outcomes), and signs it.

Submit for Review

The caregiver submits the record. It goes to their supervisor to approve — or send back with a note — and the visit is officially documented.
